請專任教職員工於本年度12月19日(星期五)前完成報名,俾利統計桌數。
一、旨揭餐會訂於115年1月15日(星期四) 中午假員生餐廳及其周邊舉行。
二、調查對象:旨揭餐會當日在職之專任教職員工(含專案研究/工作人員)。
三、符合上開條件之同仁,不論是否出席,請惠予依下述說明填報調查(逾期未填報者,除新到職之外,將視同不出席餐會,亦不製發摸彩券)::
(一)出席餐會:請於用膳欄勾選「葷」或「素」,並應如期出席。
(二)不出席餐會:請於用膳欄勾選「無需用膳」,原則不製發摸彩券。因公假、公差、因公留守、派駐校外無法出席者,請另行填寫「因公無法參與名冊」,將依名冊製發摸彩券。
四、除因公無法參與者應於摸彩券背面載明事由及員工、單位主管簽名或核章後委請同仁投入摸彩箱外,其餘參加同仁均應親自投入摸彩券並在場,凡抽中獎項經唱名3次未到者,一律視同放棄,該獎重抽。
五、擬報名參加旨揭活動之專案研究/工作人員,務請配合於年度結束前預先完成換約手續,新進同仁亦請盡快完成簽約程序,俾登入系統完成報名作業。

Full-time faculty and staff are requested to complete their registration by December 19th (Friday) of this year to facilitate the counting of tables.
I. The banquet is scheduled for noon on Thursday, January 15, 2026, at the Yuansheng Restaurant and its surrounding area.
II. Survey Subjects: Full-time faculty and staff (including project researchers/staff ) on duty on the day of the dinner .
III. Employees who meet the above criteria, regardless of attendance, are requested to complete the survey according to the instructions below (those who fail to complete the survey by the deadline, except for newly hired employees, will be considered as not attending the dinner and will not be issued raffle tickets):
( a ) Attending a meal : Please check " meat " or " vegetarian " in the meal section and attend as scheduled.
( ii ) Those who will not attend the dinner : Please check " No meal required " in the meal section . Raffle tickets will not be issued in principle. Those who are unable to attend due to official leave , business trip , official duty , or being stationed off-campus should fill in a separate " List of Those Unable to Attend Due to Official Duties " and raffle tickets will be issued according to the list.
IV. Except for those who are unable to participate due to official business, who should write the reason on the back of the raffle ticket and have their employee or supervisor sign or stamp it before entrusting a colleague to put it into the raffle box, all other participants should put in the raffle ticket in person and be present. If anyone who wins a prize does not appear after the names are called three times , they will be deemed to have forfeited the prize, and the prize will be redrawn.
5. Project researchers/staff members who intend to participate in the activities outlined above are requested to complete their contract renewal procedures before the end of the year. New employees are also requested to complete the signing process as soon as possible so that they can log in to the system to complete the registration.
